Nurse Practitioner/Physician Assistant - Orchard Park

UBMD Primary Care is seeking a full‑time Nurse Practitioner or Physician Assistant to join our Orchard Park Clinic (3875 N Buffalo St, Orchard Park, NY 14127). The APP will provide primary patient care under physician supervision, focusing on high‑quality care, health promotion, and disease prevention. Responsibilities include patient assessments, interpreting diagnostic tests, documenting medical histories, diagnosing and managing conditions, prescribing medications with physician approval, and ordering tests such as x‑rays, EKGs, and labs.

Candidates must be graduates of an accredited NP or PA program with a current NYS license; two years of healthcare experience—preferably in outpatient or primary care—is preferred. Strong communication, multitasking, and organizational skills are required, and EMR experience is strongly preferred. New graduates are encouraged to apply. 

This full‑time role offers a schedule set by practice management with no holidays, weekends, or nights required. Salary range is $104,000–$120,000, depending on the number of hours worked per session. Employment is contingent upon a background check and drug screen. AA/EOE

Job Summary:

The Advanced Practice Provider (APP) provides primary and episodic care to patients under the supervision of a physician.  The APP functions as a member of the clinical team in maintaining patient flow and maximizing the delivery of quality care to patients, while emphasizing health promotion and/or disease prevention through patient education and coordination of care. 

Essential Functions:

  • Examines patients to obtain information about their physical condition.
  • Interprets diagnostic test results for deviations from normal.
  • Obtains, compiles and records patient medical data, including health history, progress notes and results of physical examination.
  • Makes tentative diagnoses and decisions about management and treatment of patients.
  • Prescribes therapy or medications with physician approval.
  • Administers or orders diagnostic tests such as x-rays, electrocardiograms and laboratory tests.
  • Performs therapeutic procedures, such as injections, immunizations, suturing and wound care and infection management.
  • Instructs and counsels patients about prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living and health maintenance.
  • Provides physicians with assistance during complicated medical procedures.
  • Observes patients, updates charts and reports back to physicians.
  • Follows Patient Centered Medical Home (PCMH) standards.
  • Emphasizes team-based approach to care.
  • Accumulates all relevant information prior to making job-related decisions.
  • Presents well-considered alternatives when making recommendations.
  • Provides patients with educational materials, distribution of resource literature from insurance carriers and community service recommendations.
  • Coordinates timely follow up of patient requests for services regarding prescription requests, referrals, diagnostic testing and appointments for sick visits.
  • Follows regulations and guidelines for writing prescriptions.
  • Refers patients to proper resources for UBMD Family Medicine (such as billing support).
  • Independently problem solves and utilizes resources to obtain patient information when patient is unable to communicate.
  • Represents UBMD Primary Care by displaying a respectful and caring manner with patients and their families. 
  • Maintains competence through continuing medical education and in service training.
  • Excellent attendance and punctuality.
  • Reviews and addresses daily tasks as assigned.
  • Complies with all quality assurance, HIPAA, billing, PCMH, infection control, safety, OSHA and other policies/regulations.
  • Adheres to HIPAA and confidentiality policies and procedures.

Other Functions:

  • Attends regular clinic and/or provider meetings as requested.
  • Reviews company email on at least a daily basis in order to receive and address notifications and/or other pertinent information in a timely manner.
  • Any other duties as requested or assigned by the Medical Director and/or other management representative.  Duties, responsibilities and activities may change or new ones may be assigned at any time with or without notice.
  • May be required to travel to other UBMD Primary Care location(s) dependent on company need.
  • Work hours may fluctuate depending on company/clinic needs. 

Qualifications:

Education: Graduate of accredited Physician Assistant or Nurse Practitioner program.  Current NYS license as Physician Assistant or Nurse Practitioner certified by the NYS Education Department.

Experience: Minimum of two years’ experience in health care delivery, preferably in an outpatient clinic setting. 

Knowledge, Skills & Abilities: Excellent communication, flexibility, multi-tasking and organizational skills required.   Experience using an EMR system strongly preferred.  Knowledge of information and techniques needed to diagnose and treat human disease, illness or injury including, but not limited to, treatment alternatives, drug properties and interactions, and preventive health care measures.

Working/Environment Conditions:

  • Position is in a well-lit, fast-paced, clean office environment. 
  • Office noise level will be mild to moderate most times.
  • Moderate/average indoor temperatures.
  • Travel to various sites via use of personal vehicle, as requested.
  • May be exposed to occupational health hazards including infectious diseases, hazardous drugs, sharps injuries, etc.

Equipment:
